It's hard to say... since it kind of sounds like the reports are from different cycles, going by the tone of all of them. And just going by the memories we get about them, they're just memories of a small part of all of them... it's hard to paint a clear picture of what exactly occurred in them all.

But at least one report flat out mentioned it, so we can say it appeared one other time.
