The Realm's always here, right? It doesn't just vanish when it's not housing a bunch of random people. So, is it possible it's been de-stabilised and re-stabilised before, in the past?

Yes. We cannot always pay attention to everyone at all moments, but we are often aware of what you do, to some extent.
